- Original Certificate of Employment (COE) Indicating Position, Compensation and Length of Service
- 3 Months Latest Payslips & ITR (if loan is below P3.0M, ITR may be waived)
If OFW - Seafarer:
- Copy of Latest Contract from POEA and Certificate of Employment Issued by Manning Agency
- Copy of Latest 3 Mos Wages Account or Payslips
If OFW Land-based
- Original Certificate of Employment (COE) with email address of supervisor or HR Indicating Position, Compensation and Length of Service
